  • Contact Full Name: Provide the name of the primary contact for the account.

  • Address, City, State, Zip Code: This information is critical for 911 emergency services. Providing incorrect details can delay emergency response and may result in liability for fees or charges.

  • Main Phone Number: Enter the primary phone number associated with this location.

  • Additional Numbers: List any other numbers you’d like to port to our service.
